Participant Waiver Statement
By signing up for a Day of Caring volunteer opportunity, in accordance with the spirit of volunteerism and service, I, the undersigned, assume full and complete responsibility for any injury or accident that may occur to me during Days of Caring 2020 or while I am on the premises of any Heart of Missouri United Way member agency or other community organization. I hereby release and hold harmless the Heart of Missouir United Way, member agencies, community organizations and all sponsors, persons, and entities associated with Days of Caring 2020 from liability for injuries and damages sustained to me, whether caused by negligence of the sponsors, other persons, or entities associated with this event, or otherwise. I am 18 or over and agree to abide by the rules set forth in the above agreement.
I also grant Heart of Missouri United Way permission to use any photographs taken of me while I am working on the Days of Caring projects, in conjunction with any of its publicity programs.
